Deposit, Refund, & Cancellation Policy

Be Slim Bariatrics LLC (BSB) deposit, refund, and cancellation policy is posted on the health questionnaire, the consent form, patient invoice, and many other electronic forms of communications. This policy applies to all patients with any surgery type at BSB.

Deposit and Booking Bariatric Surgery:

Non-Refundable Security Deposit: A deposit is due at the same time as scheduling your surgery appointment. The deposit must be $500 USD or larger. The deposit is not refundable.

Remaining Balance: Fourteen (14) days prior to surgery, the patient needs to pay the remaining balance and must have a zero balance by this time to be seamless for the patient’s surgery date. Late payment may cause cancellation, reschedule of appointments, or additional fees. If a patient cancels or postpones the surgery date, our refund and cancellation policy will govern the remaining balance.

Refunds and Cancellation Policy: Security Deposit & Remaining Balance Deposit
Due to the cost to prepare for your surgery appointment all refunds will be handled as such (i.e., hotel reservations, hospital reservations, scheduling transportation, nutritional consultation, surgeon consultations, scheduling of hospital staff, supplies ordered based on surgery schedule, etc.):

• If the surgery is rescheduled or canceled due to a medical need discovered by the medical team during the pre-operative period or the surgery process and it is determined that the surgery cannot proceed safely, a partial refund minus the non-refundable deposit and any cost incurred by Be Slim Bariatrics will be provided. This will include but not be limited to hotel reservations, hospital reservations, scheduling transportation, hospital staff costs, supplies costs, nutritionist consult, surgeon consults, preoperative testing, etc.
• If the patient cancels more than thirty 30 days before their surgery date, Be Slim Bariatrics LLC will refund all monies paid after Be Slim Bariatrics LLC deducts the Non-Refundable Deposit fee, refund processing fee, and all expenses incurred by Be Slim Bariatrics LLC to schedule your surgery.
• If the patient cancels within 11 – 29 days before their surgery date, Be Slim Bariatrics LLC will refund up to Seventy Five (75%) of the Total Cost after Be Slim Bariatrics LLC deducts the Non-Refundable Deposit fee, refund processing fee, and all expenses incurred by Be Slim Bariatrics LLC to schedule your surgery.
• If the patient cancels within 0-10 days before their surgery date, All payments to Be Slim Bariatrics LLC, including deposit and payments towards surgery total balance, are subject to full forfeiture. Refunds are limited in this term because of the financial investment of pre-paying and in making arrangements for the hotel, transportation, surgery operating room, hospital room, administrative fees, equipment procurement, etc.
• If surgery is canceled because the patient fails to comply with Be Slim Bariatrics requirements (pre­-operative instructions, payment, consent forms, documentation, etc.), Be Slim Bariatrics LLC will refund up to Twenty-five (25%) of Total Cost after Be Slim Bariatrics LLC deducts Non-Refundable Deposit fee, refund processing fee and all expenses incurred by Be Slim Bariatrics LLC to schedule your surgery. The above policy superseed this.
• All cancellation rules and policies apply to the ORIGINAL scheduled date as well as future scheduled dates if surgery was rescheduled.
• Be Slim Bariatrics must receive all surgery cancellations or reschedules via reschedule form. 
• If collections through an agency are necessary, the fees incurred by Be Slim Bariatrics LLC will be applied to your balance owed.

Fully-Paid Patients & Remainder Funds Policy

Patients must fully pay all remaining balances before traveling to Mexico for the procedure(s). Patients must pay the remaining balance 15 days before their surgery date. If a patient fails to pay for the procedure fully, Be Slim Bariatrics LLC has the right to cancel the surgery, and all payments made for their outstanding balance will be forfeited.

Patient No-Communication Policy

If the patient fails to submit full payment 14 days before surgery date and/or fails to respond to Be Slim Bariatrics LLC’ communication attempts in regards to surgery, Be Slim Bariatrics LLC reserves the right to cancel the surgery date. In the event this happens, the patient will still owe Be Slim Bariatrics LLC for any outstanding balances and will lose their booking deposit and any payments made to their balance.

Failed Payments, Outstanding Balances Policy

If a payment fails to clear, for any reason, there will be a $50 one-time fee added to your outstanding balance. If bill collections are necessary, the costs associated and incurred by Be Slim Bariatrics LLC will be paid by the patient.

Example of Possible Additional Fees:
Our packages are all-inclusive as specified in pricing tables; however, there are times that a patient requests or may need additional items not included in the package prices. Any procedure, the doctor deems necessary to save your life or to ensure minimal complications postoperatively, during this procedure and you agree to pay for the costs related to these procedures. This includes, but is not limited to:

High BMI Fees

$525 BMI 49 to 58.9
$1,350 BMI 59 to 68.9
$1,850 BMI 69 -78.9
$2500 BMI 79+

Facilities

Market Rates plus bariatric upcharge – hotel room per night
$750 an additional night in the hospital

Equipment Fees

C-PAP – $300 (for 2 nights)
$1,500 Equipment recovery cost for Hepatitis or HIV patients

Procedure Fees

$525 Endoscopy
$525 Hiatal hernia repair *
$1,000 Umbilical hernia repair
$1,000 Gallbladder Removal in surgery
Gastric Balloon Removal – $1,500 or more
Blood Transfusions – TBD
*30% patients do need hiatal hernia repair be prepared at time of surgery just in case this is needed. Price may differ for stand alone procedures.

Hospital Fees

Covid-19 Testing $35 – $150
Intensive Care Unit – $350/Night
Cardiologist Exam/Consultation – TBD
Extra Medicines – TBD
Oxygen Treatments – TBD

Third Companion

$300/Night per companion (this applies to 2 or more companions as the first companion is included in the package)
This includes – 1. Added hotel costs for third person in room the night of arrival. 2. Hotel for each night the patient is in hospital 3. Added driver fee for pick up from airport and drop off to airport. Cost for a companion from hotel to hospital is not included.

While fees outside of the package are rare. Other medical needs are subject to fees, these are just examples as to fees that may occur outside of the package.

Refund Processing

Refunds may take up to 2-4 weeks depending on the nature of the refund. Refund will be issued to payment method on record. A processing fee of $45 or more will be applied to all refunds. This is dependent on refund processing administrative needs and any fees incurred to process a refund using the payment method on file.
